Baylor St. Luke’s Senior Care Center’s multidisciplinary approach to care puts social workers, nutritionists, nurses, mid-level practitioners, and physicians under one roof to fulfill every need of older patients.
Our care team offers longer-than-average appointment times. This allows us to fully address each person’s physical and mental health, as well as evaluate the psychosocial and dietary needs of every patient and educate them on proper medication management. An on-site social worker helps patients and their families identify resources in the community, cope with caregiver burden, and assist with future planning and advance directives. We can also help patients obtain the durable medical equipment they need to remain healthy at home.
