How can I obtain a copy of my current medical record? 

CHI St. Luke’s Health gives you, or your authorized representatives, several options for accessing your health information:

• Call our Medical Records Department at 832-355-2077.
• View portions of your medical information via MyChart.

 

How can I secure a copy of my child's (or my own) birth certificate?

Birth certificates are available only through the Houston Department of Health and Human Services Bureau of Vital Statistics. The office is located at:

​​What is the difference between a birth certificate and a birth record?

A birth certificate is an official government document that comes from the state. A birth record is an unofficial document that CHI St. Luke's Health provides to new parents as a souvenir that can be suitable for framing. Copies of the birth record can be obtained through our Medical Record department at each hospital.

How can I obtain copies of medical records from previous visits to CHI St. Luke's Health?

To protect our patients' privacy, we don't transmit confidential patient information via e-mail. The best way to obtain copies of your records is to call our Medical Record department at 832-355-2077 during business hours, Monday - Friday (8:00 a.m. - 4:30 p.m.). One of our representatives will be happy to assist you.

To obtain a copy of your medical records, you will need to complete this form. You can deliver the form in person to a CHI St. Luke's Health Medical Records Department located at one of the following hospitals, or you can mail it. IMPORTANT: There may be a charge for your records, so we strongly encourage you to call and speak to one of our representatives before mailing in the above form.
